Level Three: Promote hospice and palliative care for Veterans.
To become a Level Three Founding Community Partner (Dignity Memorial) or Community Partner:
- Sign in to your Partner Portal and complete Your Activity Report
- Upon receipt and approval of the completed Activity Report, WHV staff will:
- Email a Level Three Founding Community or Community Partner Certificate of Recognition
- Email a Level Three Founding Community or Community Partner WHV logo
- Update your location’s name on the WHV Partner Directory.
Level Three Partners may immediately begin working on Level Four Activities.
