We Honor Veterans, originally designed for hospice providers, is pleased to extend its partner opportunities to other community-based health care organizations providing direct care for seriously ill Veterans and their families.
WHV Founding Community Partners (Dignity Memorial) and Community Partners may include funeral homes, hospitals, skilled nursing facilities, assisted living facilities, home health agencies and other providers that understand the importance of serving Veterans based on their preferences for care and services.
The WHV Founding and Community Partner program has four levels of distinction based on organizations’ involvement and activities. The goal of accomplishing these levels is to provide the best quality of care to those who have served our country.
Becoming a WHV Community Partner will help ensure your organization is better prepared to:
- Build professional and organizational capacity to provide quality services for Veterans and their families
- Develop partnerships with hospices, VA, Veteran organizations and other healthcare organizations
- Promote hospice and palliative care for Veterans
- Demonstrate the impact of organizational programs

Community Partner Levels
As a We Honor Veterans Founding Community Partner (Dignity Memorial) or Community Partner, there are four levels your location can achieve. Each level guides you through specific activities to progressively increase your ability to serve Veterans and their families. The primary goals of each Partner level are described below:
Recruit: Get oriented and commit to the We Honor Veterans program
Level One: Build professional and organizational capacity to provide quality services for Veterans and their families
Level Two: Develop and/or strengthen partnerships with hospices, VA, Veteran organizations and other healthcare organizations.
Level Three: Promote hospice and palliative care for Veterans.
Level Four: Demonstrate the impact of organizational programs through evaluation and analysis of programs.
“Earn your stars” and matching logo by completing activities for each of the four levels of commitment. This enables Veterans, their families and community members to easily identify your commitment to offer Veteran-specific services provided by a competent and highly skilled workforce.
As a Founding Community Partner (Dignity Memorial) or Community Partner, you may promote your level of commitment to Veterans by displaying the We Honor Veterans Community Partner logo on your website, as well as community outreach and educational materials. As your location progresses through the four levels, you will also receive a personalized Certificate of Recognition to be proudly displayed in your facility.
