Level Four: Demonstrate the impact of organizational programs through evaluation and analysis of programs.
To become a Level Four Founding Community Partner (Dignity Memorial) or Community Partner:
- Log in to the Partner Portal and complete your Activity Report
- Founding Community Partners, submit the following to hospice.relations@sci-us.com:
- Service Enhancement Plan
- Best Practice
- Upon receipt and approval of the completed Activity Report, WHV staff will:
- Email a Level Four Founding Community or Community Partner Certificate of Recognition
- Email a Level Four Founding Community or Community Partner WHV logo
- Update your location’s name on the WHV Partner Directory.
