In this webinar, Dr. Kimberly Garner MD, JD, MPH, FAAFP, CHCQM, is joined by Elise Passy, MPH, MAEd, CDP for a discussion of the prevalence, presentation, and care of Veterans with dementia at end of life.
Dr. Kimberly Garner MD, JD, MPH, FAAFP, CHCQM is an experienced geriatrician and palliative care physician who is the South Central Veteran Healthcare Network lead for Rehabilitation and Extended Care. She is also an Associate Professor in the Department of Geriatrics at the University of Arkansas for Medical Sciences (UAMS). She is the program lead for the Advance Care Planning via Group Visits program which is a Diffusion of Excellence Gold Status Practice that is being implemented throughout the Department of Veteran Affairs.
Elise Passy MPH, MAEd, CDP is the Director of Health Systems at the Alzheimer’s Association. Elise is one of two Alzheimer’s Association Health Systems Directors for Texas and the national Veterans Health Administration lead. As the Health System Director of the Alzheimer’s Association, Elise supports health systems, physicians, clinics, and payors throughout the state, improving early detection and quality care and support for those with dementia. She works with each organization to drive risk reduction and early diagnosis, maximize quality care, and support research. Her work includes assessing health systems’ readiness, capacity, needs, and interests and providing services to support desired results. Elise is also a Certified Dementia Practitioner.

- The Program of General Caregiver Support Services (PGCSS) is one of two programs within the Caregiver Support Program. PGCSS provides peer support mentoring, skills training, coaching, telephone support, online programs, and referrals to available resources to caregivers of Veterans. The Veteran must be enrolled in Veterans Affairs (VA) health care and be receiving care from a caregiver in order for the caregiver to participate. Caregivers who participate in PGCSS are called General Caregivers. General Caregivers do not need to be a relative or live with the Veteran.
